At our clinic in Draper, we want to make it as convenient as possible for you to check in and see a physical therapist. You can complete the necessary paperwork before you even enter the clinic for your appointment. Click the links in the next column to access PDF versions our patient forms. You will need to print them and complete the forms with signatures in the appropriate places. You may:

• bring the completed forms along with your insurance card with you to your first visit,
• fax them to us at 801-523-1843 along with a copy of your insurance card (front and back),
• or scan the completed forms at your computer and email the forms along with a copy of your insurance card to us at info@southmountainrehab.com
